We need your assistance and understanding of our billing, collection, and payment policies:
1. Payment for service is due at the time services are rendered, unless payment arrangements have been approved in advance. We accept cash, checks, money orders and credit cards. Returned checks and balances older than 30 days may be subject to additional charges.
2. The Company retains the right to accept insurance assignment in certain cases. Acceptance of insurance will be dependent on an eligibility card provided by the customer and verification of coverage with the insurance company.
3. In the case of private coverage insurance the customer may be responsible for the entire balance if the insurance carrier denies payment. In the case of Medicare the customer will be held responsible for co-payments and deductibles.
4. The Company also retains the right to accept assignment on secondary or supplemental policies in certain cases. We will notify you in advance if we will accept assignment on the secondary or supplemental policy. However, if we agree to bill your secondary or supplemental policy, you will still be held financially responsible for charges that may be denied.
5. In some cases, the Company will not accept assignment on charges that are considered too small to incur reasonable billing cost. We will always notify you of these cases in advance.
6. The customer has the right to inquire how the Company obtained insurance authorizations or denials. Upon request, copies of these documents can be made available.
7. The customer has the responsibility of notifying the Company of any changes that may affect insurance carriers.
